What “ultra-processed” means
The most useful modern distinction in food is not fat versus carbohydrate but how heavily a food has been processed. The widely used NOVA framework sorts foods into unprocessed or minimally processed, culinary ingredients, processed foods, and ultra-processed foods — industrial formulations built largely from substances extracted from foods, combined with additives, and retaining little of the original structure.
The category is descriptive, not a verdict. What makes processing matter is that “how processed” predicts how much people eat better than the nutrient label does.
The trial that changed the conversation
A controlled feeding trial in which participants lived on-site shows the effect unusually cleanly. Two diets were matched for the calories presented, and for sugar, fat, fibre and macronutrients; they differed only in how processed the food was. On the ultra-processed diet, people spontaneously ate roughly 500 kcal per day more and gained weight, with the reverse on the unprocessed diet.
Because everything else was held equal, the trial attributes the difference to processing and food form rather than to any single nutrient. It is the strongest causal evidence in this area, from a small but tightly controlled study.
Why it is easy to overeat
Several features tend to travel together in heavily processed food: high energy density — a lot of calories in a small, easy-to-eat volume — softness that speeds up how fast it is eaten, and a nutrient mix, often lower in protein and fibre, that does less to trigger fullness. The protein-leverage idea captures part of this: when the protein share of a diet is diluted, total intake tends to rise as people eat more to reach a protein target.
None of this requires a food to be unusually sugary. The controlled trial matched sugar between the diets and still found the effect, which points at the whole package — energy density, form and speed — rather than at one villain nutrient.
Liquid calories and the satiety gap
Calories in drink form are among the easiest to consume without compensating for later. Solid food triggers chewing, stomach stretch and hormonal fullness signals that a drink largely bypasses, so the same energy taken as a beverage tends to add on top of the day’s intake rather than displace food elsewhere. This is the same energy-density and satiety mechanism the ultra-processed evidence points to, applied to the most effortless form of all.
A limit: this rests on the general energy-density and body-weight-regulation literature rather than on a single beverage trial, and it describes a mechanism rather than giving advice about any particular drink.
Fibre, grain quality and what got left out
A diet built on ultra-processed food is not only a diet with more of certain things; it is a diet with less of others, and the clearest casualty is fibre. Refined grains replace whole ones, and the intact structure that feeds gut bacteria and slows digestion is milled away. Across pooled studies, whole-grain intake is associated with lower cardiovascular disease, cancer and death from any cause, and whole grains compare favourably with refined grains against the same outcomes.
So the modern diet’s problem is partly one of displacement: the more room heavily processed food takes up, the less is left for the fibre-rich whole foods that carry the most consistent signal in all of nutrition.
